В статье представлен опыт успешного хирургического лечения хронического послеоперационного остеомиелита грудины и ребер, развившегося после коронарного шунтирования. Стратегия хирургического лечения гнойного осложнения состояла из радикальной хирургической обработки гнойного очага передней стенки грудной клетки, дополнительной ультразвуковой обработки раны и после перехода раневого процесса в репаративную стадию – пластической реконструкции передней стенки грудной клетки комплексами местных мягких тканей. Описанное клиническое наблюдение является примером внедрения в практику стратегии активного хирургического лечения обширных гнойных ран применительно к хроническому остеомиелиту грудины и ребер.
The paper describes the authors, experience in surgically treating chronic postoperative sternal and costal osteomyelitis developing after coronary bypass surgery. The strategy in the surgical treatment of a purulent complication consisted of radical surgical debridement of a purulent focus in the anterior chest wall, additional ultrasound wound treatment and, after the transition of wound healing to the reparative stage, plastic reconstruction of the anterior chest wall with complexes of local soft tissues. The described clinical case is an example of the practical introduction of the strategy in the active surgical treatment of extensive purulent wounds and ribs, as applied to chronic osteomyelitis of the sternum and ribs.
