Лечение пролежней – одна из сложных задач гнойной хирургии у соматически тяжелых пациентов с нарушенным кровообращением в зонах, подверженных длительному давлению, трению или сдвигу тканей. Методы хирургического лечения зависят от локализации пролежней и общего состояния. При этом процент вынужденных инвалидизирующих операций при пролежнях, осложненных инфекционным процессом, остается высоким, что также требует дальнейшего изучения.</p><p>Цель исследования – анализ эффективности методов хирургического лечения пролежней разной локализации.</p><p>Материалы и методы исследования. Изучены результаты хирургического лечения 105 пациентов с пролежнями разной локализации в возрасте от 16 до 76 лет. Из них у 11,4 % пациентов отмечено множественное поражение. В 39,0 % наблюдений пролежни локализовались в крестцовой области, 25,7 % – в седалищной области, 14,3 % – в области большого вертела, 3,8 % – в области гребня подвздошной кости, 2,9 % – в лопаточной области, 1,9 % – в пяточной области, 1,0 % – в затылочной области.</p><p>В зависимости от фазы течения раневого процесса все больные были разделены на 2 группы. В 1-ю группу вошли 45 (42,9 %) пациентов с пролежнями в I фазе раневого процесса. Всем им проводили радикальные и этапные хирургические обработки ран с иссечением некротизированных тканей, адекватное дренирование. После очищения раны и появления грануляционной ткани 32 (71,1 %) пациента поступили на второй этап лечения – закрытие дефектов, у оставшихся 13 (28,9 %) больных пролежни зажили вторичным натяжением самостоятельно.</p><p>Во 2-ю группу вошли 60 (57,1 %) пациентов с пролежнями во II фазе раневого процесса и 32 (30,5 %) пациента после первого этапа операции.</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования. В процессе лечения 18 (19,6 %) больным проведены хирургическая обработка и санация раны с последующим наложением вторичных швов или аутодермопластика полнослойным кожным трансплантатом, а 7 больным –резекция седалищной кости, после чего отмечено полное заживление ран вторичным натяжением. При этом 67 (72,8 %) пациентам выполнена пластика перемещенными ротационными кожно-жировыми лоскутами, кожно-фасциальными и кожно-фасциально-мышечными ротационными лоскутами на сосудистой ножке.</p></sec><sec><title>Заключение</title><p>Заключение. Хирургическое лечение пациентов с пролежнями разной локализации с учетом фазы течения раневого процесса рекомендовано как наиболее эффективный и безопасный метод. Пластика пролежней различной локализации во II фазе раневого процесса перемещенными ротационными кожно-жировыми, кожно-фасциальными и кожно-фасциально-мышечными лоскутами на сосудистой ножке в сочетании с активным дренированием раны являются наиболее эффективными методами.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Treatment of pressure sores is one of the intractable tasks of purulent surgery in somatically severe patients with impaired blood circulation in areas subject to prolonged pressure, friction, or tissue shear. Surgical treatment methods depend on the location of the pressure sores and the general condition. At the same time, the percentage of forced disabling operations for bedsores complicated by the infectious process remains high, which also requires further study.</p></sec><sec><title>Objective</title><p>Objective: to substantiate the effectiveness of surgical treatment of pressure sores of various localization.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The results of surgical treatment of 105 patients with bedsores of various localization aged from 16 to 76 years were studied. Of these, 11.4 % of patients had multiple lesions. About 39.0 % of patients were case had bedsores localized in the sacral region, 25.7 % – in the sciatic region, 14.3 % – in the area of the large trochanter, 3.8 % – in the area of the iliac crest, 2.9 % – in the scapular region, 1.9 % – in heel area, 1.0 % – in the occipital region.</p><p>Depending on the phase of the wound healing process, all patients were divided into 2 groups. The first group included 45 (42.9 %) patients with bedsores in the inflamation phase. All of them underwent radical and stage-by-stage surgical debridement of wounds with excision of necrotic tissues, adequate drainage during treatment. After cleansing the wound and the appearance of granulation tissues, 32 (71.1 %) patients were admitted to the second stage of defect closure surgery, the remaining 13 (28.9 %) patients had bedsores closed by secondary tention.</p><p>The second group included 60 (57.1 %) patients with bedsores in the second phase of the wound healing process and 32 (30.5 %) patients after the first stage of surgery.</p></sec><sec><title>Research results</title><p>Research results. During treatment, 18 (19.6%) patients underwent surgical treatment and rehabilitation of the wound, followed by secondary sutures or autodermoplasty with a full-layer skin grafts, and 7 patients underwent sciatic bone resection, after which complete healing of bedsores was noted. At the same time, 67 (72.8 %) patients underwent plastic surgery of bedsores with displaced rotational skin-fat flaps, skin-fascial and skin-fascial-muscular rotational flaps on the vascular pedicle.</p></sec><sec><title>Conclusion</title><p>Conclusion.
